That could well be the case.  In metro areas people want their local traffic reports.  In rural areas ther is no traffic, only 5 stations broadcasting more or less the same thing.  In metro areas listeners have as much as 80 stations between AM and FM here on the Atlantic Coast.  Why pay for more?  Rural listeners would seem to me to be the most deprived and, therefore, the most apt to go for pay radio.

It would be interesting for somebody to do their thesis on the geographic distribution of satellite radio listeners and what this says about listeners.
