Sorry to hear of your misfortune. Have you looked at the electo-larynx 
devices? They are a small hand held device that you hold to your throat that makes a 
humming noise, and as you mouth the words, it modulates the humming so it 
comes out as speach. Fits in the palm of your hand. I have been taking care of a 
patient at the hospital who has been on a ventillator for an extended amount 
of time and it works great. No guessing what the patient wants (too weak to 
write).
