[IRCA] form-maker software for logsheets

Folks,

I am going back to hand-written logs like I used in olden days, simply 
because updating logs via PC means generating new pages every few loggings.

I don't keep my DX den in the same room as my workroom where our PC is; to 
keep down RFI, so readout from a display screen is not an option.

Also, I have very neat block handwriting.

You can see where I'm going:

I'm looking to generate printed logsheets, and for that I would like to 
software to print vertical lines on
lined school paper, and also a top horizontal border and a box in the upper 
right-hand corner for the frequency.

The question follows:

Does anyone know of an inexpensive form-generating software that I can use?

Let's keep on topic, here. I don't want to be getting advice to get a 
plasma screen monitor or a laptop. Let's be reasonable.
