I don't think I'm old enough to remember the KMLO calls.  I do regularly hear Vista day and night, though (currently KCEO).  Signal is fair - about an S4 (the way I judge the RST scale) on a typical portable - enough to light a tuning indicator or trip a seek/scan, in the daytime.  At night it's weak and competes with KOMO Seattle, although usually KCEO comes out on top.
Also, I don't typically count call changes or format changes as new loggings, but I will count transmitter & antenna changes as new loggings.
For example, KDIS and KSPN in the Los Angeles area swapping frequencies a number of years ago isn't grounds for new loggings for me.  However, local KCBQ moving to a different transmitter site with a different power & antenna configuration would for me technically be a new logging, even though they had the same callsign and format.

KAVL can be heard here in the day with a VERY selective radio & a sensitive antenna, although IBOC from local 600 KOGO makes it extremely difficult.  (It is possible to DX near local IBOC, though, as I've heard 594 JOAK Tokyo, Japan, out from between 600 KOGO and its IBOC, whose 5kW is about 7.7 miles west of me.)  At night, though, they're a regular, with KEAR San Francisco also making an occasional appearance.

KURS is a regular day and night here.  At night there is signs of fading from co-channel / skywave interference, but KURS is always on top of the channel.  (My next logging of WHO from here will be my first, although I've heard them from Redlands, CA.)  Also KURS shares a tower with 1240 KNSN (formerly KSON) and that GY signal is good enough to own the frequency at night.

I don't remember if I've heard KMAP.  My daytimer on 1050 is XED from Mexicali (although KCAA Loma Linda is also possible) and at night I often hear KTCT San Mateo.

Don't know if I've heard KAFY here.  I do regularly hear KFAX San Francisco and the ESPN outlet (IIRC) from Las Vegas, NV.

Don't remember KSDT, but I do hear KKSM Oceanside in the day here.  At night it's usually the one from Sacramento, IIRC, although I haven't checked in a while.

I don't remember if I've heard KWXY at night in the jumble here.  In the daytime, KCLU Santa Barbara dominates from 195 miles away, as heard in this recent clip recorded with the PL-606 and Select-A-Tenna:

I got something on 920 one early afternoon when local 910 KECR was off the air.  I'm not sure if it was Palm Springs or Ensenada, though.  I don't think I've logged 1010 there, and splatter from 1130 KSDO is too much for even my Tecsun PL-606 to dig out much of anything on 1140.  My best Coachella Valley signal, at least in the daytime, is 970 KNWZ.

I have a semi-local on 1550, although around SR/SS, KUAZ Tuscon also makes a regular appearance.  I don't remember what I've had on 1590, although I suspect Ventura, Mexicali or Ensenada would be more likely possibilities for me.
