The ones I use simply put the radio audio closer to the ears.  There is
still more than enough ambient sound coming through.  I often drive with
earplugs to keep things down a bit.  My truck is somewhat noisier than a
car.  All it does is bring the clatter down to the point where my ears don't
ring so loudly after a long drive.  It's pretty much equivalent to turning
the radio up to overcome road noise.
