As far as I can tell, the "attachment" is simply the unsubscribe
instructions.  Do you have those in the body of your message?

The last entry is "14.  In Performance".  If you have that, you have
all the material of relevance.

Here's what the attachment says, best as I can figure -- Outlook
Express opened it inline.
